Tax and financial planning for elder care
How families in India can think about tax, savings and paperwork when paying for an older parent’s care, with official links and cautions.
Elder care in India is usually paid for out of several pockets at once: the older person’s savings and pension, contributions from working children, sometimes siblings splitting the bill, occasionally a scheme or an insurance claim. The tax and paperwork side gets neglected until the return-filing season, and then families discover that records were never kept.
This page explains how to organise the money side. It is general information, not advice. Tax figures and rules change yearly — confirm the current year’s rules with a chartered accountant.
Start with the record, not the deduction
Open one folder — physical or digital — for the financial year and keep four things in it: medical bills and pharmacy receipts, insurance premium receipts, home-care invoices, and bank statements showing who paid. That is the raw material for a return, an insurance reimbursement, or a claim under any scheme.
If three siblings share the cost, decide early whether one pays and the others reimburse, or each pays a bill directly. Either is fine; a muddle is not. Note the arrangement in a short email that everyone replies to.
What the income-tax rules are trying to do
The Act makes distinctions by age, and the thresholds for a senior citizen and for an even older bracket are not the same. It also allows certain deductions for health insurance premiums and for specified medical expenditure for senior citizens, and separate relief for the maintenance of dependants in defined circumstances. Each of these comes with conditions: who is covered, whose income it relates to, what proof is needed, and a ceiling.
All of it is as provided by the Act — check the current year. The reliable text is at https://www.incometax.gov.in, and a chartered accountant will tell you in ten minutes whether your situation qualifies. Never plan around a figure you saw in a social media post.
Interest, pensions and filing
Interest income is taxed differently for older taxpayers than for others, and tax may be deducted at source on interest depending on the declaration you give the bank. Pension received is income. Retirement corpus withdrawals can have their own treatment. If your parent’s total income falls below the taxable threshold, a return may still be worth filing to establish a record and to claim refunds of tax deducted at source.
Filing season runs through the monsoon for most individual taxpayers. File on time even if no tax is due; a late filing attracts consequences that are not worth avoiding an hour of work for.
Keep every document that supports a figure within reach: interest statements, premium receipts, pharmacy bills and home-care invoices, filed by year. Most queries from the department arise because a number could not be traced back to a document, not because the number itself was wrong.
Who pays, and whose expense it is
A common family question is whether an adult child can pay a parent’s medical bill and set it off against their own income. Sometimes yes, sometimes no, and it turns on the specific provision and its wording. Where money moves within a family, there is also the question of gifts, clubbing and documentation. Getting this checked once, in writing, is much cheaper than an adjustment notice later.
For families abroad, add currency conversion, remittance proof and the question of who is assessed for tax in which country — see caring for NRI parents.
What the money actually has to cover
Plan costs before planning relief. As an indicative placeholder pending local survey work, home attendant care is often quoted in the region of ₹15,000–₹40,000 a month, with skilled nursing, physiotherapy and equipment billed on top — see the cost guide and the money and cost catalogue for how those bands are built. A year of that cost is the number your savings need to absorb, deduction or no deduction.
Keep a separate liquid buffer — six to twelve months of care costs is a common family rule of thumb — so that a hospital admission does not force a premature deposit withdrawal.
Support you should check for anyway
Central assistance for elderly people in households meeting published criteria runs through the National Social Assistance Programme at https://nsap.nic.in, and the Ministry of Social Justice and Empowerment at https://socialjustice.gov.in is the nodal point for much elder welfare. Whether a household qualifies is determined only by the department administering the scheme.
The practical next step, once the money side is arranged, is to work out what care is actually needed at home — the caretaker matcher takes you through that.
FAQ
Common questions
Can I claim a deduction for money I spend on my parents?
It depends on the nature of the expense and whose income it relates to. Some provisions allow deductions for health insurance premiums or medical expenditure for senior citizens, and some for the maintenance of dependants, but each has conditions. Nothing is automatic. Confirm the current year’s rules with a chartered accountant and read the text on https://www.incometax.gov.in before you plan around a figure.
Should we put the care costs on one person’s name?
Keep a simple, consistent record of who paid what, and make sure the person paying is the one claiming any relief they are entitled to. A shared family spreadsheet of receipts and bank entries saves arguments later and makes the return straightforward to file. Where several siblings contribute, agree the arrangement in writing at the start.
When should we talk to a chartered accountant?
Before the financial year closes, not during the return-filing rush. Rules, thresholds and forms change each year, and the planning that works is the one built on the year that is actually in force. An accountant can also tell you quickly whether a proposed arrangement is allowed at all.
